1. Bath City were a Conference side and sharing their ground with Brizzle Rovers in 1996 (AWS semi-final game)
2. Would this have had anything to do with the restructuring of the Premier League? Or possibly adjustments made when Maidstone folded in 1992?
Cant help you with the other two sorry ;D

I think the 5 relegation spots were because the premier league went from 22 to 20 clubs in the mid 90's.

Question 4 if you are in Telford 12 Years. Everywhere else in Shropshire 11.
The Telford Combination has 2 Divisions The West Shropshire just one.
If AFC Telford had started at the bottom it would have been the following . . .
Telford Combination First Telford Combination Prem Sportsjamkits.com Shropshire County League First Division Sportsjamkits.com Shropshire County League Premier Div West Midlands League Division 2 West Midlands League Division 1 West Midlands League Premier Midland Alliance Southern League Western Southern Premier Conference South Conference National
It depends where you are in the country and what Pyramid agreements were in place.

Bath City played at Alliance Premier level on at least 2 occasions.
The first was in about 1987-8 season and then they were relegated. They came back again in about 1990-91 season and stayed around for a couple of years.
They had Alan Churchward in goal if I remember rightly.
Noted for their chant of "Bath Spa, Ra Ra Ra"

What happens to the team who finish bottom of the 12th league? and how do teams join in that league or get promoted? Bottom club (poss 2) apply for re-election at the AGM. It is usually only a formality and they are voted back in. Teams are always folding, so if you apply before the end of one season you will be considered in the summer and allowed in for the next season, as long as everything is ok (pitch, money etc.) Further details from the Shropshire FA, I imagine
