Not trying to be condescending but did you leave any zeros out of your DOB? for example writing 01.01.2000 instead of 1.1.2000? might be the issue as i have just done it and it has found me straight away. Fair point, it would be an easy mistake to make but, no, I entered the zeros. Just tried again with and without spacing in my NHS number - neither worked. Can't believe that a week ago it got me my vaccination and now I'm not recognised. It's bad enough that my consent is assumed so that I have to take action to opt out, but it's even more annoying how difficult they're making it for people to exercise their choice. You may have been categorised as an unperson. Watch your step, you may be scheduled for Vaporization and erasure from society. Taken out of books, photographs, and articles so that no trace of you is found in the present anywhere – no record will be found. This is so that a person who defies the Party will be gone from all citizens' memories.

In a small way, it does highlight how the individual is lost in the sweep of big data. If all the ducks are in line, great, everything works swimmingly. Take a step out of line though, whether or not it was you consciously taking the step, and you're on your own.

If any of you carry a smartphone and are moaning about this you haven't a leg to stand on. Nevermind share your data, your phone listens to you and tracks everything you do. Fk it then, might aswell give all my info away to the highest bidder. Having a smartphone and allowing access to your medical data are similar incursions into personal privacy but not the same thing. The notion that you've effectively signed away your right to ownership of your personal data because you have a smartphone is nonsense.

Update: NHS Digital still doesn't recognise me, either by NHS number or postcode, but I rang them and they told me that's a common problem at the moment. They'll send the form to fill in if it can't be done online. They might take a couple of weeks to arrive because of how busy they are but the start date has been put back to 1 September (from 1 July), so that shouldn't be a problem.
You can include up to six people from one household on a single form.
